SERN Programme and Woodland Wellbeing Sessions

SERN Programme and Woodland Wellbeing Sessions

The Diabetes Team at the Alex currently cares for around 250 children and their families, which includes approximately 40 young adults who will shortly be transitioning to the adult services diabetes team.

We have agreed to fund four SEREN (Structured, Education, Reassuring, Empowered and Nurturing) educational programmes to be run locally, away from the hospital, where these young adults can learn to manage their diabetes into adulthood alongside this developmentally and socially challenging period and meet the adult services team who will then take over their care when they are discharged from the RACH.

We are also funding six further Woodland Wellbeing days for children who are newly diagnosed with diabetes and in need of peer support.

Thanks to Little Lives for funding this project.
